Vaccine Schedule

Age appropriate vaccination is extremely important to the physicians of this practice. Below, please find our typical vaccine schedule. Remember, check-ups are done annually after the age of 2 years (regardless of whether or not vaccines are anticipated at that age). In accordance with ACIP, CDC and AAP recommendations, we also recommend annual flu vaccines.

  • Birth—Hepatits B
  • 2 weeks—None
  • 6 weeks—DTaP, HIB, Hepatitis B, Rotateq
  • 3 months—Prevnar, IPV, Rotateq
  • 4 months—DTaP, HIB
  • 5 months—Prevnar, IPV, Rotateq
  • 6 ½ months—Prevnar, DTaP, HiB
  • 9 months—Hepatitis B, IPV
  • 12 months—HIB, MMR
  • 15 months—Varicella, Prevnar
  • 18 months—DTaP, Hepatitis A
  • 2 years—Hepatitis A
  • 3 years—None
  • 4 years—DTaP, IPV
  • 5 years—MMR
  • 6 years—Varicella
  • 11 years—TdaP
  • 12 -18 years—Gardasil, Menactra
