Appointments

We understand occasional circumstances prevent you from keeping your scheduled appointment. If possible, please notify our office in advance if you are running late or unable to make your appointment. Our scheduling coordinators will do their best to re-schedule you promptly.

Insurance and Billing

We are in network with the following PPO insurance plans: Aetna, Anthem Blue Cross, Blue Shield of California, Cigna, Healthnet, and United Healthcare. We are also in-network with Blue Shield EPO and UNited Healthcare EPO and POS. EPO and PPO. We are not in-network with HMO plans.

Payment Options

This office currently accepts credit card, cash or check for payments. All co-payments are due at the appointment time. We reserve the right to refuse service when patients do not make their co-payments.
