We are located in the COTTON NORTH building.
Parking
Parking is located in the parking lot at the end of Alessandro Street on the left side of the street. It is a paid parking lot, and we do NOT validate. Once there, you can go up the elevator in the Cotton SOUTH building up to the first floor or take the stairs on your left-hand side up one flight to the first floor. Cross the street and enter the Cotton NORTH building and proceed to the 2nd floor, Suite 201.
There is also limited street parking on Alessandro Street and Bellefontaine Street.

Appointments
We understand occasional circumstances prevent you from keeping your scheduled appointment. If possible, please notify our office in advance if you are running late or unable to make your appointment. Our scheduling coordinators will do their best to re-schedule you promptly.
Insurance and Billing
We are in network with the following PPO insurance plans: Aetna, Anthem Blue Cross, Blue Shield of California, Cigna, Healthnet, and United Healthcare. We are also in-network with Blue Shield EPO and United Healthcare EPO and POS. EPO and PPO. We are not in-network with HMO plans.
Payment Options
This office currently accepts credit card, cash or check for payments. All co-payments are due at the appointment time. We reserve the right to refuse service when patients do not make their co-payments.
