What to expect
A pleasant, comfortable first visit builds trust and helps put the child at ease during future visits. We want your child to enjoy getting to know our doctors and staff, so we work hard to establish this bond during every appointment to our office. During your initial appointment, we will explain everything in detail and answer any questions you may have.

Patient Forms
To expedite your first appointment, please arrive a few minutes early to complete registration forms so that we have all the necessary information to treat your child. You may also download and print the forms from this website, fill them out ahead of time, and bring them with you to the first appointment.
Please complete all forms prior to your first visit to our office. All forms are required.

Insurance and Billing
We are in network with the following PPO insurance plans: Aetna, Anthem Blue Cross, Cigna, Healthnet, and United Healthcare. We are also in-network with Blue Shield EPO and United Healthcare EPO and POS.
PLEASE NOTE: We are awaiting Blue Shield of California PPO to put us in network. We will update the website once we have received word back from them! Thank you for your patience!!!
We are not in-network with HMO plans.
Parking
For parking, the main parking lot is on Alessandro Street; you will head to the end of the street and turn left into the parking entrance. The parking lot is pay parking with a max of $7 per day. We do NOT validate. From the parking structure, go up the stairs on the left side to head to the Cotton North building which is across the street. When you enter the building, you will see a pharmacy on your right side and Quest diagnostics on your left.
Payments
We accept cash, credit cards and checks. If you have an outstanding payment, please PAY HERE.
